If you are a patient of Legacy Health residing in the United States (1) who was invited to and did register for a Legacy Health patient portal account from February 18, 2019, through December 31, 2020 (“Category 1”), and (2) do not fall within Category 1, but otherwise logged into Legacy Health’s Patient Portal from January 1, 2021, through February 9, 2024.You may submit a Claim for a Settlement benefit, outlined below.
If you are a Class Member and wish to receive Settlement Benefits then you must submit a claim form by March 16, 2026.
Legacy Health will establish a Settlement Fund of $2,200,000.00. The Settlement Fund will first be used to pay Court-approved Attorneys’ Fees and Costs, Service Award payments for the Plaintiffs, and Settlement Administration Costs. The net remaining money will be used to pay for Settlement Member benefits.
All Settlement Members may claim one (1) year of Medical Data Monitoring services, and a Cash Payment. The benefits are explained in more detail below.
Medical Monitoring Services: All Settlement Members were sent a code and link for redeeming (1) year of CyEx Medical Shield Complete product, free of charge and without the need to submit any Claim. This product includes the following features:

The code and link for your Medical Shield Complete was previously sent to you as part of the email Notice of this Settlement. If you have misplaced your Notice containing the code and link, please contact the Settlement Administrator at (833) 647-9039.
Cash Payment: All Settlement Members may make a claim to receive a cash payment of $15.00.
To start your online claim form, you must login with your Unique ID and Claim Login PIN. You can find your ID and PIN on the Notice that was sent to you. If you are unable to locate your ID & PIN, please contact the Settlement Administrator by emailing info@PixelLegacyHealthSettlement.com. Please request your ID & PIN by providing your full name and mailing address.
